‘Working Creatively with Risk’ – #OTalk 30th April 2013.

This weeks #OTalk will look at risk, how we assess, manage and work with risk.

Most if not all organisations will include this in induction training and will have adopted there own practices and paperwork etc. So we will cover these issues briefly. However, the focus of the chat will be how we as Occupational Therapists work with risk and more importantly ‘how creative are we when exploring individual needs and wishes?’

Below are a few resources to get the thinking going:

COT Guidance for members can be accessed here: Risk Management

The Health and Safety Executive have many resources to guide your thinking and practice around Risk management and advocate some very useful guidance about the Principles of sensible risk management.

Sensible risk management is about:

  • Ensuring that workers and the public are properly protected
  • Providing overall benefit to society by balancing benefits and risks, with a focus on reducing real risks – both those which arise more often and those with serious consequences
  • Enabling innovation and learning not stifling them
  • Ensuring that those who create risks manage them responsibly and understand that failure to manage real risks responsibly is likely to lead to robust action
  • Enabling individuals to understand that as well as the right to protection, they also have to exercise responsibility

Sensible risk management is not about:

  • Creating a totally risk free society
  • Generating useless paperwork mountains
  • Scaring people by exaggerating or publicising trivial risks
  • Stopping important recreational and learning activities for individuals where the risks are managed
  • Reducing protection of people from risks that cause real harm and suffering
Understanding how the HSE will evaluate decisions can also be helpful HSE’s decision-making process: Reducing risks, protecting people.

Risk Culture, appetite and tolerance.

What type of risk culture do you work in?

Is this congruent with your own risk culture or that of our profession?

The Institute of Risk Management considers ‘An effective Risk culture to be one that enables and rewards individuals and groups for taking the right risks in an informed manner’.

Risk Appetite and Tolerance are concepts that are aimed at organisations and have been developed to help executive boards and directors build an appropriate risk culture. The Institute of Risk Management’s view is that ”both risk appetite and risk tolerance are inextricably linked to performance over time. We believe that while risk appetite is about the pursuit of risk, risk tolerance is about what you can allow the organisation to deal with”.

What are your feelings about risk?

Do you work collaboratively with your clients to assess and manage risk?

Who makes the final decision?

What have feelings got to do with risk?

Do you and your organisation support risk to facilitate positive outcomes for an individual?

The following examples are taken from a social care view point with a focus on personal budgets, included here to spark some thoughts and discussion about risk and how we balance the positive and negative aspects of risk and risk taking.

Social care TV: Personal Budgets: Risk Enablement and Mental Health

At a glance 31: Enabling risk, ensuring safety: Self-directed support and personal budgets

Key messages

  • Providing real choice and control for people who use social care means enabling people to take the risks they choose, particularly in the use of self-directed support and personal budgets.
  • With the support of frontline staff, people using services should be enabled to define their own risks and to recognise, identify and report abuse, neglect and safeguarding issues. Informed choice is vital.
  • Practitioners may be concerned with balancing risk enablement with their professional duty of care to keep people safe.
  • Practitioners need to be supported by local authorities/Northern Ireland health and social care trusts to incorporate safeguarding and risk enablement into relationship-based, person-centred working. Good quality, consistent and trusted relationships and good communication are particularly important.
  • Risk enablement can transform care, not just prevent abuse. Risk enablement and safeguarding training for staff, people using services, carers and families is important in achieving this.
  • Risk enablement should become a core part of placing people at the centre of their own care and support. It cannot be a ‘bolt-on’ solution to traditional adult social care systems which are not person-centred.

I look forward to exploring these issues and learning from your experiences, tweet you on Tuesday 8pm (UK) as usual.

@Nnikki_Duffy #occhat reading can be social, work, relaxation, emotional. So given that if a person puts reading as an interest what would u ask? #occhat (Tue, 23 Apr 2013 20:53:44 +0100)

@BillWongOT @AbbyPediatricOT @Nnikki_Duffy Now we just need an occupational scientist who is willing to study all that. #occhat (Tue, 23 Apr 2013 21:22:23 +0100)
@AbbyPediatricOT @BillWongOT @Nnikki_Duffy for example, parents of children with autism or adults with a cancer diagnosis. #occhat (Tue, 23 Apr 2013 21:22:39 +0100)
@AbbyPediatricOT @BillWongOT @Nnikki_Duffy Reading would probably provide meaning for both groups, but in very different ways. #occhat (Tue, 23 Apr 2013 21:23:18 +0100)
@BillWongOT @AbbyPediatricOT @Nnikki_Duffy or you can start planning a study to present it at WFOT in 2018. #occhat (Tue, 23 Apr 2013 21:23:41 +0100)

#occhat 23rd April 2013 – World Book Night – Reading as an Occupation

Well tonight is unprecedented – all of the #OTalk/#occhat crew are busy elsewhere so dear colleagues we are relying on you to keep the chat going.

Inspired by my (Kirsty) alternative evening plans tonight’s #occhat will be about Reading as an occupation.

Today is World Book Night where a team of volunteers (including myself) are giving out free books to non or light readers.

This is the third year I’ve taken part and this year I’ve decided to give out my books closer to home, to OT students in fact. I will be giving out Malorie Blackman’s Noughts and Crosses in the hopes that the students can give themselves permission to take time out and immerse themselves in a book for pleasure.

I have recently become more serious about reading and it has made me wonder why I ever let it slip from my priorities. But I know that not everyone is a fan of reading so this is where tonight’s chat comes in.

Some questions to guide your discussion:

Are you a reader? Why? Why Not?

What do ‘get’ from reading?

When do you read?

Do you have a special reading place?

Does the form reading take change its meaning for you? e.g. reading for work, reading poetry, newspapers, reading a ‘book’ versus an e-book?

What do you read? Do other people’s opinions of the type of books you read matter?

Does reading for you stop with that book or do you need to talk to other people about it?

Do you/have you belong(ed) to any book clubs, face to face or virtual, Goodreads?

Of course feel free to add in your own questions to each other and chat away.

Don’t forget the #occhat in your posts, please help newcomers and remember your code of conduct.

(If my book event finishes early enough I will try and join you but please do chat away without us – we will still grab the chat and summarise on the blog as usual).

Part 3 Will explore – Self care – supporting people to understand and manage their own conditions.

The first two #OTalks provided much for Occupational Therapists to consider, as a little reminder the introductory blogs can be found here:

Part 1: Using Risk Stratification to case find people with Long Term Conditions

Part 2: Integrated health and social care neighbourhood care teams – supporting people with Long Term Conditions

#beddingout for #occhat 9th April 2013

Tonight, #occhat are discussing a fantastic artwork that is being created this week by Liz Crow(@RGPLizCrow), with support from Dawn Willis (@Quinonostante). Liz will be Tweeting through Dawn to take part in the chat- its a great opportunity to ask her questions ahead of the 48 hour performance art extravaganza.

The work has relevance and importance, relating to narratives of disability and empowerment: what the lives of people demonised in the media as “benefit scroungers” actually represent: and how our occupations (such as creating art) play into mainstream understanding of disability and social security, and how they enable us to create our own story and promote our agency in a world still full of barriers. Join us at 8pm BST on 9th April 2013 to talk about this, on the #occhat tag as usual, and the project will be collated on the #beddingout tag over the next few days.

Liz has helpfully provided this blog about the work, in her own words:

At the beginning of April, I am moving body and soul into bed for 48 hours round-the-clock, exhibited behind a red rope barrier in a public art gallery

Bedding Out is a response to the welfare benefits overhaul, which threatens many with poverty and with a propagandist campaign that has seen disability hate crime leap by 50%.

The work looks at the way I live a life in two very separate parts. There’s a public self that tries to be outgoing and animated and changing the world, and most people assume because that’s what they see of me, that’s how I am in the rest of my life. But then there’s the private self, which wins no friends or accolades, in which I spend most of my time at home, a lot of time lying down and quite a lot in bed.

This is the self that I have become very expert in concealing. And whilst that has kind of worked for the past 30 years, in the face of benefits changes, it no longer does. Instead, this new system demands that I reverse myself, parading the private me to justify support.

In the performance I take this private self and make it public, performing my bed-life. Since the public me is so carefully constructed, this is a kind of un-performing of my self. I want to make visible a twilight existence shared by thousands of us. But even more, I want to show that what so many people see as contradiction – what they call fraud – is just the complexity of real life.

Bedding Out can portrays the human story set within its broader social context. The work is not just about me, but is a symbol for the thousands of us who live a bed-life. There is something about playing it out in a public space that goes deep to the heart of what that life means, setting it overtly within the present, very difficult political, climate.

At points throughout the performance, members of the public will be invited to Bedside Conversations, gathering round the bed to talk about the work, its backdrop and its politics. Previous conversations ranged far and wide across benefits, newspaper propaganda, hate crime, art as activism, and much more.

You can get a flavour of the work from ‘Reflections from the Bed’ ’http://www.roaring-girl.com , short audio-visual slideshow (with captions) that tells more about the work and why it feels so necessary.

And because, at last autumn’s version of this performance, I was contacted by several people delighted that the work was making them more visible, but too ill to attend in person, this time, Bedding Out will use social media to involve absent people into the conversation.

How you can get involved

Via the web: www.roaring-girl.com

Watch Bedding Out! The performance will be live streamed throughout the 48 hours at www.roaring-girl.com.

Bedside Conversations (duration 40 minutes) will be live streamed with audio, BSL interpretation and live subtitles:
Wednesday 10 April 2.30pm and 6.00pm
Thursday 11 April noon (via twitter) and 3.15pm
Friday 12 April 10.15am.

On Twitter: @RGPLIzCrow #beddingout

Follow the work – and join in! We have tweetmeister Dawn Willis (@quinonostante) tweeting in the lead up to the performance and in residence throughout the 48 hours. She is tweeting live updates on the performance, audience reactions, issues of the day and Bedside Conversations, and responses to individual tweets, bringing people into the bigger conversation and encouraging them to make the project their own.

Tweets will be fed into conversations and displayed live in the arts centre alongside the performance.

There will also be an all-twitter conversation on the Thursday at noon, with over 50 people already signed up to take part.

If you’re not on twitter, you can text us. We can upload what you say to twitter. If you want to remain anonymous, just type ‘MySecret’ before your text. (Outside the UK take away the 0 and add 44 to the number).

In person:

If you’re in Salisbury, come to the Arts Centre! Opening hours and access information at: http://www.roaring-girl.co.uk/productions/bedding-in/show-times/

My hope is that people will come to see Bedding Out, pitch in via social media, use it to talk to people new to the issues raised, use it beyond the gallery space and after the performance is over. I hope the work will be part of the much larger conversation of disabled people, supporters, the Sparticus campaign, DPAC, Black Triangle, UKUncut and others, our combined voices reaching more people and generating public outrage to force a change in benefits reform and cultures of work towards a system that is humane.
